Instructions: Human Rights Response Form

Use this form to respond to a human rights complaint you received from the Alberta Human Rights Commission (the Commission, we, us). Any personal information collected on the form is in compliance with section 4 of the Protection of Privacy Act.

Important information

  • The Commission must receive your response within 30 days from the date you received the notification letter.
  • Please respond carefully and thoroughly. This helps everyone involved and will make it easier to process the complaint.
  • There is more information about the complaint in the notification letter and copy of the Complaint Form that we sent to you. You can also read the Responding to a complaint page.
  • If there is a signed release that covers the complaint allegations, or if the complaint falls within federal jurisdiction only, you do not need to respond to the allegations in the complaint at this time. If this applies to you, provide supporting information in Section C. We may ask for additional information and submissions later.

Filling in the response form

  1. Using this online form is the preferred method to submit your response. There is a PDF version available if you are unable to use this online form.
  2. This form has 10 steps. Your information will not be saved if you close your browser. If you need to pause while completing the form, just leave the browser tab open. The Commission website will not time out. However, some browsers may have a security setting that overrides this.
  3. The Commission will only receive your form once you press Submit at the end of the form. At the same time a copy of the completed form will be sent to the email address you provide.
  4. Items marked with an * must be filled in.
  5. Contact the Commission if you have questions about completing the form.
  6. At the end of this form, you can attach a file. You can upload up to 12 pages. If Section C applies to you, it has a limit of 10 additional pages.
