Purpose of this form

If you want someone to act as an authorized representative for you during the complaint process, this form applies to you. It allows the Commission to communicate with the representative directly, instead of you.

  • An authorized representative means a person who is not a lawyer, but who has been authorized by a complainant or respondent to represent them in the complaint process. An authorized representative could be an advisor, support person, relative, friend, or human resources professional.
  • A lawyer is a person who is allowed to practice law in Alberta. A complainant or respondent must pay the costs of a lawyer themselves. The Commission does not give legal advice. Lawyers do not need to complete this form, but should provide a letter confirming their retention.
  • Complete this form if you did not already provide your representative's information in your Complaint Form or Response Form. You can file this form at any time in the complaint process.

Any personal information collected on the form is in compliance with section 4 of the Protection of Privacy Act.

Other situations need a different form

You will need a different form if you are a litigation representative. A litigation representative is a person who represents a minor under 18 or a person who lacks legal capacity to participate in the complaint process. Filling in this form

  1. Using this online form is the preferred method to tell us about an authorized representative. There is a PDF version available if you are unable to use this online version.
  2. This form has 3 steps. Your information will not be saved if you close your browser. If you need to pause while completing the form, just leave the browser tab open. The Commission website will not time-out. However, some browsers may have a security setting that overrides this.
  3. The Commission will only receive your form once you press Submit at the end of the form. At the same time, a copy of the completed form will be sent to the email address you provide.
