Forms

If you want to make a complaint of discrimination, you may find it useful to complete a self-assessment. Answer a series of questions to see if your complaint would likely fall within the Alberta Human Rights Act.

Instructions on how to complete the Complaint Form are in the Human Rights Complaint Guide. Review the Guide before completing the Complaint Form.

There are PDF versions of the forms available if you are not able to use the online forms below.

Canada Post service disruption: The Commission cannot send or receive physical mail through usual methods, including registered mail, for the duration of the Canada Post Strike.

If required, you can send mail to the Commission using a Government of Alberta mail drop location.

Alternatively, correspondence can be sent by email, fax, and courier, or dropped off to our office, in person.

*Please note, the date of receipt for complaint forms submitted using a mail drop location will be the date the Commission receives the form, not when you drop it off.
